Summary/Goals Tab

This topic only applies to Ontario pharmacies.

The Summary/Goals tab is where the outcome of the medication review is documented. Multiple topics about the summary and goals can be documented.

Upon completion of the medication review, the fields in this tab automatically populate the Summary and Goals section of the Pharmacist Worksheet form and the Patient Take-Home Summary form. Pharmacists may provide patients with the Patient Take-Home Summary that is intended to further engage patients in identifying ways to build added awareness in their drug therapy and help to identify therapy-related goals.

If completing a diabetes medication review, the Patient Take-Home Summary form will be titled Diabetes Education Take-Home Summary form.

To complete the Summary/Goals tab:

  1. Enter information in the following fields:

    • Summary of Today's Discussion

    • Patient Goals/My Goals

    • What I will Do to Get There/Reaching the Goal

    • List of Resources and Contacts Provided

    • Other Follow-Up Planning and referrals/Referrals Information

  2. In the Notes field, enter any non-patient facing comments, as required.

  3. Select Add Note to add the note.

  4. Select the plus button to add additional goals. A new Topic tab appears at the top of the Summary/Goals tab.
